Associating an Embed Form at the Stream Item Level

This document will show how you can set a Form to be displayed at the Stream Item level.

Forms can be set at a Hub level, where one Form is shown for all the Assets on your Hub, or you have the ability to set a particular Form for each Asset in a specific Stream.

To add a specific Form to an individual Asset, follow the steps below:

1. From the Left Navigation Menu, click Hubs.
   
2. On the Hubs listing page, select a Hub.
   
   
3. From the Hub Menu, click Streams.
4. On the Streams listing page, select a Stream. On the Stream details page, under the Contents tab, hover over the asset, and then click the Edit icon (gear).     
5. On the Edit Item Pop-up window, scroll down to the Form field, and then select the Form from the Form drop-down list (for example a Form with a Marketo Embed). Click Save.
   

How the Forms appear on your Hub Stream Item level:

When a visitor views any of the Asset that has a Marketo Embed Form is configured at the Stream Item level, the Asset will displayed with the Embed Form. 

In this case, the Marketo Embed Form is displayed at the end of the Asset.

Once the email is submitted through the Embed Form, the lead is directly created in your Marketo account. With this process, the lead will not go through the Hushly Lead Enrichment Process.
